Indiana State License Application

1. Carefully complete license application. You can obtain one from your school or directly from the Division of Professional Standards (previously IPSB).

2. All applications require a Limited Criminal History report, which you obtain from the Indiana State Police Central Repository. The original document you receive must accompany your application and be dated within the past 11 months. To obtain the Limited Criminal History Report:

Send a $7.00 certified check or money order made payable to the "State of Indiana" with a self-addresses, stamped envelope and the "Request for Limited Criminal History Information" form (from your Education or Speech Pathology/Audiology Department at school) to:

The report must be returned to you for you to send with your license application. The Central Repository will not send the report to a third party i.e. Ball State.

OR

You can go to the address above and the Limited Criminal History Check will be done while you wait. You will need your valid driver's license and $7.00 cash.

3. Original Instructional licenses require completion of the Praxis I: PPST and Praxis II: Subject Assessment tests. Ball State uses the ASHA examination. Test scores must be on file at the Educational License Office.

4. Applicants with a dual major or minors will need to complete two license applications. You can make a copy of the attached application.

5. Original teaching, administrative and school service licenses require a certified check or money order for $35.00 made payable to the "State of Indiana." Dual majors will require two $35.00 money orders or certified checks.

6. License applications should be mailed to the Educational License Office at your college or university.

7. Licenses are mailed directly to the address on your application.
